Does the market incentive and transparency bring about Long-term care(LTC) quality improvement?: Focusing on the quality maintenance and transition from quality grading systems

Abstract
The quality management of long-term care (LTC) facilities applied to LTC evaluation in order to strengthen the regulations based on market in South Korea. The study aims to examine the effects of market-based quality improvement tools from the OECD market-based analysis framework on the current LTC quality grade maintenance and deviation from the grade system (e.g., close or reestablish facilities) among LTC facilities. This study set up panel data merging public resource and organizational survey data. Also, ordinal and binomial logistic model were used in the study. In the result of the study, good quality grade maintenance of LTC facilities was related to the occupancy fulfillment rate, the number of user waiting for admission, previous quality grade, non-profit organization, size of organization, and age of organization. On the other hand, employees¡¯ turnover rate and the change of social worker¡¯s rate as an additional staff were not significantly related to the current quality grade maintenance. Rather, the lower the number of third-grade users in LTC and low-income users were associated with the higher quality grade maintenance. Second, deviations from the current quality grade system were found to be more relevant to the reduction of occupancy fulfillment rate, the decrease in the proportion of social worker rate, and the private organization. Therefore, the results was interpreted that LTC facilities which were good at quality maintenance responded to policies that emphasized market principles, but they did not consider user protection such as third-grade users and low-income users who did not have compatible condition in the market competition. This study pointed out the limitations of market-based quality management policies in LTC and suggested political recommendations for the better LTC quality maintenance.
